Abstract

Two large, randomized trials, one in the United States and the other in Europe, have determined that carotid endarterectomy prevents stroke in patients with neurologic symptoms and severe stenosis of the internal carotid artery. The North American Carotid Endarterectomy Trial found the cumulative risk for stroke ipsilateral to stenosis to be 24% in medically treated and 7% in surgically treated patients at 18 months after randomization. The European Carotid Surgery Trial similarly found a lower risk in its surgical than in its medical arm.
