Risk of renal dysfunction in critically ill trauma patients receiving aminoglycosides

Abstract

Critically ill patients are at particular risk for developing acute renal failure. More than 50% of all cases of acute reversible renal failure are related to surgery or trauma. Ischemia secondary to major surgery, hypovolemic shock, or septic shock is often the cause of acute renal insufficiency in these patients. Bacteremia also may cause direct renal tubular damage in the traumatized patient, producing a synergistic effect when combined with an aminoglycoside. The incidence of renal dysfunction in critically ill trauma patients receiving aminoglycosides is poorly defined. Because many potential risk factors for developing aminoglycoside-induced nephrotoxicity are found in trauma patients, we chose to review retrospectively the medical records of all patients admitted to a trauma intensive-care unit with documented or presumed infection who received at least four doses of an aminoglycoside. The objectives of this study were to (1) assess the incidence of decreased renal function in trauma patients receiving an aminoglycoside and (2) identify possible risk factors for development of renal dysfunction in these patients. Recognition of these risk factors would be a valuable aid in selecting an aminoglycoside for use in a given patient and in determining the degree of monitoring for nephrotoxicity after initiation of therapy.
