Transcranial and Cervical Ultrasound in Stroke

Abstract

Transcranial Doppler (TCD) and cervical duplex ultrasound (CDU) offer insights into acute stroke mechanisms and provide real-time information for clinical decision making in acute stroke patients, selection for, and monitoring effects of therapeutic interventions. Both tests can be repeatedly performed at bedside. Main limitations include operator dependency of ultrasound testing technique and absence of trans-temporal window particularly in older patients. Nonetheless, TCD and CDU are applicable to the vast majority of stroke patients, and successful practice of TCD and CDU requires in-depth knowledge of anatomy, physiology, scanning techniques, and interpretation principles that have to be incorporated during neurology residency and vascular neurology fellowship training. This chapter provides overview of key diagnostic applications for TCD and CDU tests.
